新規抗凝固薬とその周辺

・NOACs

※Dabigatranは投与後12-24HのaPTTがULMの２倍以上であれば、出血リスクが高いかも．あくまで、qualitative evaluation!!!!

※坑Xa因子ではPTがqualitative evaluationになりえる。。。

・適応

・術前の休薬期間

・出血時の対応

・のみ間違いの時のRecommendation

　 −飲み忘れた時はuntil halfway of dosing interval (1x/d なら12時間まで)なら飲む

　−それ以上遅れてしまった時は、次回をスケジュール通りに飲む

　−間違えて２回飲んでしまった時は、次をスキップする選択肢もあり

　−飲んだかどうかわからない時は、

　  BIDならその時点で飲まずに、12時間後から予定通りに

　  QIDなら、その時点で一錠のんでその後予定通りに
